    The Research on Establishment of “Clinical Practice Guide of Blood Specimen Collection, Preservation and Delivery for Clinical Nurse”: Protocol Description

    INTRODUCTION AND OBJECTIVE: The correctness of the blood test is closely related to the sample. According to the recent reported data, 80 percentage unsatisfactory results of the clinical test are due to the poor quality of sample, especially the blood sample. Clinical practice guide (CPG) is directly to instruct the clinical nursing practice. And the recommendations in the clinical practice guide are based on the best available study evidences. There is lack of CPG about blood sample specimen collection, preservation and delivery (BSCPD) in China. Additionally, related published clinical studies are accumulated in a great deal. Therefore, establishing a CPG is necessarily and practicable. The detailed objectives are: 1) to describe and analyze the research status of BSCPD in China; 2) to describe and analyze the practice status of BSCPD in China; 3) to systematic appraise the available evidences of BSCPD; 4) to establish the clinical practice guide of BSCPD; 5) to judge the clinical practice guide of BSCPD. METHODS: Objective 1): Bibliometric analysis is applied, the database include China National Knowledge Infrastructure Database (CNKI) and Sino-med and the research period is from the year of 2003 to 2013; Objective 2): Questionnaire survey for all the registered clinical nurses in a grade three hospital; objective 3): Systematic review according to Cochrane collaboration handbook 5.1.0 is applied which includes assessment of risk of bias, data extraction, data analysis; Objective 4) and 5): Using the appraising guidelines research and evaluation (AGREE) to evaluate of the draft of CPG of BSCPD. RESULTS: Bibliometric analysis started in 2013, and search strategies have been established. Questionnaire survey setting and depth interviewees have been identified and communicated. CONCLUSIONS: The CPG of BSCPD will serve as an important resource in instructing and modifying clinical nursing practice. Given this CPG of BSCPD will be a draft version, the applicability and suitability of it will need a further evaluated in the real clinical nursing world

    Effects of recombinant adenovirus-mediated hypoxia-inducible factor-1alpha gene on proliferation and differentiation of endogenous neural stem cells in rats following intracerebral hemorrhage

    AbstractObjectiveTo investigate the effects of adenovirus (Ad)-mediated hypoxia-inducible factor-1alpha (HIF-1α) gene on proliferation and differentiation of endogenous neural stem cells (NSCs) in rats following intracerebral hemorrhage (ICH) and the underlying mechanisms.MethodsA total of 120 specific pathogen-free, adult, male Sprague-Dawley rats were included in this study. After establishment of ICH models in rats, PBS, Ad, or Ad-HIF-1α was administered via the ischemic ventricle. On the 1st, 7th, 14th, 21st and 28th d after ICH, rat neurological deficits were scored, doublecortin (DCX) expression in the subventricular zone cells was detected by immunohistochemical staining, and 5-bromo-2'-deoxyuridine (BrdU)-, BrdU/DCX-, and BrdU/glial fibrillary acidic protein-positive cells in the subventricular zone were counted using immumofluorescence method among PBS, Ad, and Ad-HIF-1α groups.ResultsOn the 7th, 14th, 21st and 28th d after ICH, neurological deficit scores in the Ad-HIF-1α group were significantly lower than in the PBS and Ad groups (P<0.05). In the Ad-HIF-1α group, DCX expression was significantly increased on the 7th d, peaked on the 14th d, and then gradually decreased. In the Ad-HIF-1α group, BrdU-positive cells were significantly increased over time course, and significant difference in BrdU-positive cell counts was observed when compared with the PBS and Ad groups at each time point (P<0.01 or 0.05). On the 7th, 14th, 21st and 28th d after ICH, the number of DCX-, BrdU-, BrdU/DCX-, and BrdU/DCX-positive cells in the Ad-HIF-1α group was significantly greater than in the PBS and Ad groups (P<0.05).ConclusionsHIF-1α gene can promote the proliferation, migration and differentiation of endogenous neural stem cells after ICH, thereby contributing to neurofunctional recovery after ICH

    Four-leg converters with improved common current sharing and selective voltage-quality enhancement for islanded microgrids

    Four-leg dc-ac power converters are widely used for the power grids to manage grid voltage unbalance caused by the interconnection of single-phase or three-phase unbalanced loads. These converters can further be connected in parallel to increase the overall power rating. The control of these converters poses a particular challenge if they are placed far apart with no links between them (e.g., in islanded microgrids). This challenge is studied in this paper with each four-leg converter designed to have improved common current sharing and selective voltage-quality enhancement. The common current sharing, including zero sequence component, is necessary since loads are spread over the microgrid and they are hence the common responsibility of all converters. The voltage-quality enhancement consideration should however be more selective since different loads have different sensitivity levels towards voltage disturbances. Converters connected to the more sensitive load buses should therefore be selectively triggered for compensation when voltage unbalances at their protected buses exceed the predefined thresholds. The proposed scheme is therefore different from conventional centralized schemes protecting only a common bus. Simulation and experimental results obtained have verified the effectiveness of the proposed scheme when applied to a four-wire islanded microgrid
